Intricacies in Application of Non- Invasive Ventilation (NIV)
Hands-on on Non-invasive care for leading doctors in Patna, Feb 16, 2019

A one-day workshop was conducted on the application of Non- invasive ventilation in hospital and homecare environment – which was attended by more than 30 renowned and eminent chest Physicians/ pulmonologists. This one its kind workshop was held in Patna on Feb 16, 2019, at Lemon Tree Premier.
This workshop covered both theoretical and hands-on learning on the application of Non-invasive ventilation across a variety of patients in various types of respiratory failure including Acute Hypercapnic failure and Chronic Hypercapnic Failure. The use of NIV in Chronic care and NIV Guidelines were also discussed.
Non- invasive ventilation is the application of ventilator support without the use of invasive mechanism such as endotracheal intubation or tracheostomy through an interface such as full face mask which gives inspiratory and expiratory positive pressures and improves ventilation – and is very effective in managing various forms of respiratory failure, especially COPD.
COPD is a collective term used to describe progressive lung emphysema, chronic bronchitis, refractory asthma and bronchiectasis leading to breathlessness. It is one of the leading causes of deaths in India, especially during winters. A survey claimed 12 lac plus deaths in 2017-18 through both outdoor and indoor pollution. The most common causes and risk factors for COPD are smoking, second-hand smoke, fumes, chemical and dust in construction sites.
Patients benefit from NIV as it provides pressure support and which assists patient’s breathing to enable more volume to enter into their lungs in a shorter period of time. This decreases their work of breathing (WOB) which lowers their respiratory rate and allows more time to exhale.
The symposium was kick started by Dr S. K. Madhukar – consultant and interventional chest physician, Madhukar Clinic, followed by Dr Saket Sharma, Senior Pulmonologist, Indira Gandhi Institute of Medical Sciences, Dr Sanjay Kumar, Senior Chest Physician, Dr Kishore Jhunjhunwala, Director, Palika Vinayak Hospital, who are experts in the field of Non- Invasive ventilation.
Dr S. K. Madhukar spoke about the indications and Goals of NIV whereas Dr Saket Sharma discussed its role in acute hypercapnic respiratory failure. In order to spur this application, for the patient benefit or end users, the training was followed by experiential learning on NIV application, monitoring and troubleshooting by Dr. Jaydeep Khalpada, Clinical Specialist, ResMed India
The workshop offered all stakeholders in the healthcare system a platform to learn, share and connect. The workshop brings together a multi-disciplinary audience from across the healthcare spectrum to share their experiences, ideas and break the conventional silos that exist in this field.
A non-conventional ventilator contrary to the normal belief, can be used with the mask on an awake patient retaining his ability to communicate, eat and drink. This offers several advantages such as reduced sedation requirement, improved patient comfort, ambulatory ventilation, patient recovery, reduced cost to the patient and early discharge.
On the occasion, Dr Kishore Jhunjhunwala said, ‘I congratulate, ResMed Academy for conducting such educational workshop that sensitizes and improves the knowledge of upcoming doctors, hence increasing the adoption of NIV. More the doctor is trained and is aware of new technology, more it will benefit patients”.
Dr Sanjay Kumar praised ResMed Academy’s effort and said, “Earlier there was very less understanding on the COPD and many suffered due to lack of drugs and counselling. However, with the introduction of NIV in healthcare, many patients were benefited and it improved the quality of life”.
“Such symposiums should be frequently conducted as it not only benefits patients but doctor to be abreast with the latest innovations available in the market”, he added.
According to Dr Saket Sharma, improving knowledge about NIV amongst healthcare practitioners is the need of the hour keeping in mind the large number of patients who suffer from hypercapic failure who can be effectively managed using NIV.
